When you choose Save intake & continue to payment, we save your name, email, phone, state, selected concern, consent, and a submission reference in encrypted private storage on Vercel. We use these details to handle your evaluation request and contact you about next steps. Your email and an opaque intake reference are sent to Stripe for checkout; medical answers are not sent to Stripe. Submitted records remain available to authorized service operators after you close your browser. Unsubmitted answers stay in browser memory and clear on reload. Your state selection may be saved for this browser session. Advertising tags are not loaded in the intake. Optional Google Ads measurement is available on the homepage. Optional analytics and location suggestions are explained below.

Our host estimates a US state from the connection to suggest a state. We do not store the raw IP for this feature. The suggestion can be changed and does not confirm vehicle registration or eligibility. Hosting infrastructure may maintain operational access logs.
With your permission, we send anonymous numbered funnel steps, button events, numeric advertising campaign IDs, and confirmed purchase amounts and currency to PostHog to measure campaign performance. We send an anonymous analytics ID and campaign IDs to Stripe with checkout so confirmed payments can be attributed. Purchase confirmation may arrive after you close this page. We do not send these conversion events to advertising platforms. We do not send names, contact details, medical answers, full page URLs, or raw IP addresses to PostHog. Session replay and automatic field capture are disabled. Use Analytics preferences to change your choice. Anonymous identifiers last for the browser session; your analytics preference is stored on this device.
Optional Google Ads measurement
With your separate permission, our public homepage loads Google Ads tag AW-18542132043 for advertising measurement. Google may receive browser and network information and use advertising cookies. Ad personalization and user-provided data features are disabled. This tag is not loaded on intake, medical-condition, or checkout pages. We do not send names, medical answers, or purchases to Google Ads. Choose Analytics preferences on the homepage to decline or withdraw permission. PostHog analytics can be allowed separately.
